Evolution of the Health Canada astronaut biodosimetry program with a view toward international harmonization

Biodosimetry of astronaut lymphocyte samples, taken prior to- and post-flight, provides an important in vivo measurement of radiation-induced damage incurred during space flight which can be included in the medical records of the astronauts. Health Canada has been developing their astronaut biodosimetry program since 2007 and since then has analyzed data from 7 astronauts. While multiple cytogenetic endpoints may be analyzed for the astronauts, the Fluorescent in situ hybridization (FISH) assay is considered to be key for detecting long-lasting stable damage. It is believed that this long-lasting damage is most likely to lead to an increased risk to the health of the astronauts during long-term flights (lasting 6 months or more). The complexity of damage that results from protracted, non-homogeneous radiation exposure, like that found in the space environment, requires a detailed scoring schematic to capture as much information as possible. To that end, this paper outlines the efforts to harmonize the manner in which Health Canada’s FISH data are recorded to better facilitate the comparison of results with other international biodosimetry programs.

Rapid culture-independent microbial analysis aboard the International Space Station (ISS)

A new culture-independent system for microbial monitoring, called the Lab-On-a-Chip Application Development Portable Test System (LOCAD-PTS), was operated aboard the International Space Station (ISS). LOCAD-PTS was launched to the ISS aboard Space Shuttle STS-116 on December 9, 2006, and has since been used by ISS crews to monitor endotoxin on cabin surfaces. Quantitative analysis was performed within 15 minutes, and sample return to Earth was not required. Endotoxin (a marker of Gram-negative bacteria) was distributed throughout the ISS, despite previous indications that mostbacteria on ISS surfaces were Gram-positive [corrected].Endotoxin was detected at 24 out of 42 surface areas tested and at every surface site where colony-forming units (cfu) were observed, even at levels of 4-120 bacterial cfu per 100 cm(2), which is below NASA in-flight requirements (<10,000 bacterial cfu per 100 cm(2)). Absent to low levels of endotoxin (<0.24 to 1.0 EU per 100 cm(2); defined in endotoxin units, or EU) were found on 31 surface areas, including on most panels in Node 1 and the US Lab. High to moderate levels (1.01 to 14.7 EU per 100 cm(2)) were found on 11 surface areas, including at exercise, hygiene, sleeping, and dining facilities. Endotoxin was absent from airlock surfaces, except the Extravehicular Hatch Handle (>3.78 EU per 100 cm(2)). Based upon data collected from the ISS so far, new culture-independent requirements (defined in EU) are suggested, which are verifiable in flight with LOCAD-PTS yet high enough to avoid false alarms. The suggested requirements are intended to supplement current ISS requirements (defined in cfu) and would serve a dual purpose of safeguarding crew health (internal spacecraft surfaces <20 EU per 100 cm(2)) and monitoring forward contamination during Constellation missions (surfaces periodically exposed to the external environment, including the airlock and space suits, <0.24 EU per 100 cm(2)).

The nutritional status of astronauts is altered after long-term space flight aboard the International Space Station

Defining optimal nutrient requirements is critical for ensuring crew health during long-duration space exploration missions. Data pertaining to such nutrient requirements are extremely limited. The primary goal of this study was to better understand nutritional changes that occur during long-duration space flight. We examined body composition, bone metabolism, hematology, general blood chemistry, and blood levels of selected vitamins and minerals in 11 astronauts before and after long-duration (128-195 d) space flight aboard the International Space Station. Dietary intake and limited biochemical measures were assessed during flight. Crew members consumed a mean of 80% of their recommended energy intake, and on landing day their body weight was less (P = 0.051) than before flight. Hematocrit, serum iron, ferritin saturation, and transferrin were decreased and serum ferritin was increased after flight (P < 0.05). The finding that other acute-phase proteins were unchanged after flight suggests that the changes in iron metabolism are not likely to be solely a result of an inflammatory response. Urinary 8-hydroxy-2'-deoxyguanosine concentration was greater and RBC superoxide dismutase was less after flight (P < 0.05), indicating increased oxidative damage. Despite vitamin D supplement use during flight, serum 25-hydroxycholecalciferol was decreased after flight (P < 0.01). Bone resorption was increased after flight, as indicated by several markers. Bone formation, assessed by several markers, did not consistently rise 1 d after landing. These data provide evidence that bone loss, compromised vitamin D status, and oxidative damage are among critical nutritional concerns for long-duration space travelers.

Microbe-I: fungal biota analyses of the Japanese experimental module KIBO of the International Space Station before launch and after being in orbit for about 460 days

In addition to the crew, microbes also find their way aboard the International Space Station (ISS). Therefore, microbial monitoring is necessary for the health and safety of the crew and for general maintenance of the facilities of this station. Samples were collected from three sites in the Japanese experimental module KIBO on the ISS (air diffuser, handrail, and surfaces) for analysis of fungal biota approximately 1 year after this module had docked with the ISS. Samples taken from KIBO before launch and from our laboratory were used as controls. In the case of KIBO, both microbe detection sheet (MDS) and swab culture tests of orbital samples were negative. The MDS were also examined by field emission-scanning electron microscopy; no microbial structures were detected. However, fungal DNAs were detected by real-time PCR and analyzed by the clone library method; Alternaria sp. and Malassezia spp. were the dominant species before launch and in space, respectively. The dominant species found in specimens from the air conditioner diffuser, lab bench, door push panel, and facility surfaces on our laboratory (ground controls) were Inonotus sp., Cladosporium sp., Malassezia spp., and Pezicula sp., respectively. The fungi in the KIBO were probably derived from contamination due to humans, while those in our laboratory came from the environment (e.g., the soil). In conclusion, the cleanliness in KIBO was equivalent to that in a clean room environment on the ground.

Crewmember and mission control personnel interactions during International Space Station missions

INTRODUCTION: Reports from astronauts and cosmonauts, studies from space analogue environments on Earth, and our previous research on the Mir Space Station have identified a number of psychosocial issues that can lead to problems during long-duration space missions. Three of these issues (time effects, displacement, leader role) were studied during a series of long-duration missions to the International Space Station (ISS). METHODS: As in our previous Mir study, mood and group climate questions from the Profile of Mood States or POMS, the Group Environment Scale or GES, and the Work Environment Scale or WES were completed weekly by 17 ISS crewmembers (15 men, 2 women) in space and 128 American and Russian personnel in mission control. RESULTS: The results did not support the presence of decrements in mood and group cohesion during the 2nd half of the missions or in any specific quarter. The results did support the predicted displacement of negative feelings to outside supervisors in both crew and mission control subjects on all six questionnaire subscales tested. Crewmembers related cohesion in their group to the support role of their commander. For mission control personnel, greater cohesion was linked to the support role as well as to the task role of their leader. DISCUSSION: The findings from our previous study on the Mir Space Station were essentially replicated on board the ISS. The findings suggest a number of countermeasures for future on-orbit missions, some of which may not be relevant for expeditionary missions (e.g., to Mars).

Giant hepatic hemangioma and cross-fused ectopic kidney in a spaceflight participant

Commercial spaceflight participants are typically older than traditional astronauts and often have medical conditions that make medical certification for flight difficult. This case report considers a 43-yr-old spaceflight participant who planned a short-duration Soyuz flight to the International Space Station (ISS). While he participated in many hazardous activities such as parachuting, hang gliding, scuba diving, Antarctic and jungle exploration, and deep sea submersible operations, he knew that several of his medical conditions precluded serving as a career astronaut. At the time of his initial spaceflight prescreen examination, he was known to have previous bilateral photorefractive keratectomy (PRK) for myopia and a cross-fused left ectopic kidney that would be disqualifying for a career astronaut. During the evaluation for the left single cross-fused ectopic kidney, a giant hepatic hemangioma was also discovered. In order to medically qualify for flight, the giant hepatic hemangioma was surgically removed. This case summary investigat*es the implications of a single cross-fused left ectopic kidney and the decision process and treatment implications for spaceflight medical certification in an individual with an asymptomatic giant hepatic hemangioma.

International Space Station Medical Standards and Certification for Space Flight Participants

Introduction: The medical community of the International Space Station (ISS) has developed joint medical standards and evaluation requirements for Space Flight Participants (“space tourists”) which are used by the ISS medical certification board to determine medical eligibility of individuals other than professional astronauts (cosmonauts) for short-duration space flight to the ISS. These individuals are generally fare-paying passengers without operational responsibilities. Material and Context : By means of this publication, the medical standards and evaluation requirements for the ISS Space Flight Participants are offered to the aerospace medicine and commercial spaceflight communities for reference purposes. It is emphasized that the criteria applied to the ISS spaceflight participant candidates are substantially less stringent than those for professional astronauts and/or crewmembers of visiting and long-duration missions to the ISS. Conclusions: These medical standards are released by the government space agencies to facilitate the development of robust medical screening and medical risk assessment approaches in the context of the evolving commercial human spaceflight industry.
